  Step 5      
Use Shape Tool again and draw buttons apply the same blending options like on "banner" !    
      
  The Final Step      
Now just add your text, graphics, logo and more.

  Step 1      
Open a new document in photoshop ( 200 x 200 pixels ) and write down the C letter. In my case the font is Anja Eliane.    Then Rasterize Type and duplicate the layer and then go to Edit> Transform> Flip Horizontal.    
And set up like i did.
  
      
  Step 2       
Then we move the C letters together that they will look like the S letter. Like this:  
      
  Step 3       
Then select both layers and press CTRL+E that the layers integrate.     
  Step 4       
Apply these blending options...  
      
      
      
      
      
      
      
      
        
  Step 5       
Then we duplicate the layer 5 times and always when we duplicate press the key up.     
  Step 6       
Now go to Edit> Transform> Perspective and have fun and do like i did.     
      
Then type the letter G and repeat the steps 4-6.     
Finish with text added.

Razterize Layer adn then Filter--> Distort-> Ripple and set up like i did.
